The nonstop management of a chronic disease is overwhelming for patients and the people who care about them. In order to reduce the burden of emergency room visits and hospital stays, patients must be empowered with tools that allow for self-monitoring and improved decision making. We have worked with various companies and products to address this ongoing challenge.

  • How might we leverage the role of a partner in a patient’s care plan?

  • What is the value of a digital health coach for cardiac patients post-surgery?

  • How might we prepare patients for a complex medication routine?

Bridging the gap between doctors and patients can foster the support and care that is the foundation for a trusting relationship. This leads to better informed patients and caregivers who are more engaged in the difficult choices of health management. Medical information in a patient-friendly language

Improving healthcare doesn't always mean providing more education. Disease and therapy management means organizing the vast amounts of information that is distributed to us as patients. Add on the opportunities provided by the digital world and we multiply the means by which we receive that information exponentially. Patients need to be given the right information, at the right time, in the right way in order to achieve their health goals.
